You Think You Need Botox.
You Might Actually Need Skin Support First.
By Dr Rianna, Co-founder of RiKo Aesthetics | Doctor-Led Aesthetics Clinic, Hemel Hempstead
Anti-wrinkle injections, commonly known as Botox, are the most requested treatment we see at RiKo Aesthetics, and for good reason. When used correctly, at the right dose and in the right client, they are one of the most effective treatments available for softening expression lines, preventing deeper wrinkles from forming, and giving the face a more rested, refreshed appearance.
However, Botox is not always the answer. In some cases, choosing anti-wrinkle injections before addressing what the skin actually needs can leave clients feeling disappointed. They often wonder why their results do not seem to last as long as expected, or why fine lines remain visible even after treatment.
More often than you might think, the reason is dehydration. The solution is not more Botox. It is giving the skin the support it has been asking for.
This is the final article in our series on What You Think You Need vs What You Actually Need. If you have not yet read the previous articles on chin filler versus lip filler, cheek filler versus tear trough filler, and lip filler versus nose filler, they are well worth exploring. The message running through every article is the same. The face works as a complete system, and treating a single concern without understanding its underlying cause rarely delivers the result a client is truly hoping for.
Why Dehydrated Skin and Anti-Wrinkle Injections Don’t Always Mix
There is something I wish more clients understood before booking their first anti-wrinkle treatment.
Anti-wrinkle injections work by temporarily relaxing the muscles beneath the skin that create expression lines, such as frown lines, forehead lines and crow’s feet. They are exceptionally effective at treating these dynamic wrinkles. What they cannot do is improve lines that are caused by changes within the skin itself.
This is something that surprises many people. A significant proportion of fine lines, particularly the fine, crepey lines that appear across the forehead, around the mouth and beneath the eyes, are not primarily caused by muscle movement. They are caused by dehydration and declining skin quality.
When the skin lacks adequate hydration, it loses its plumpness, elasticity and natural ability to smooth itself. Fine lines that would otherwise be barely noticeable become much more visible. The skin can appear dull, crepey and older than it really is. No amount of anti-wrinkle treatment will resolve this because the muscle is not the problem. The skin is.
This is why we regularly meet clients who feel that their Botox has “stopped working” or no longer lasts as long as it used to. Their first instinct is often to increase the number of units or book appointments more frequently. Yet the fine lines remain because the skin needs hydration and collagen support, not additional muscle relaxation.
The quality of the skin also influences how anti-wrinkle treatments look. When the skin is dehydrated or lacks collagen, the results can appear to wear off sooner. This does not necessarily mean the product has become less effective. Instead, the skin no longer provides the healthy foundation needed to display the result at its best. By treating dehydration first, or alongside anti-wrinkle injections, we can often achieve results that look better, feel more natural and last longer.
The Static Lines That Botox Cannot Fix

There is another situation where improving the skin should take priority over anti-wrinkle injections, or at least be combined with them. This applies to static lines. To understand why, it helps to know the difference between dynamic and static lines.
Dynamic lines are the wrinkles that appear when you make an expression. These include the horizontal lines across your forehead when you raise your eyebrows, the frown lines between your brows when you concentrate, and the crow’s feet that appear when you smile. These lines disappear, or become much less noticeable, when your face is relaxed. Anti-wrinkle injections work exceptionally well for these types of wrinkles because they temporarily relax the muscles that create them.
Static lines are different. They remain visible even when your face is completely at rest. Over time, repeated facial movement, combined with a gradual loss of collagen, elastin and skin quality, causes these lines to become permanently etched into the skin. As the skin loses its ability to bounce back after movement, the wrinkles remain, even when the muscles are no longer active.
This is where many people become disappointed. They expect Botox to erase these established lines, but that simply is not how the treatment works. Anti-wrinkle injections can stop the muscle from making a static line worse, but they cannot remove a wrinkle that is already embedded within the skin.
To improve static lines, the skin itself needs to be repaired and strengthened. This means stimulating new collagen production, restoring hydration and improving the overall quality of the skin. Once the skin becomes healthier, these lines often soften significantly.
At RiKo Aesthetics, when we assess a client with established static lines, our recommendation is often to focus on rebuilding the skin’s foundation first. Once the skin has improved, we can then reassess whether anti-wrinkle injections are needed to prevent those lines from becoming deeper in the future.
In our experience, this combined approach consistently produces better, more natural-looking results than relying on anti-wrinkle injections alone.
The Skin Support Treatments That Make the Difference
So, what does skin support actually involve?
There is no single treatment that suits everyone. The best approach depends on your skin, your concerns and your long-term goals. At RiKo Aesthetics, we create personalised treatment plans, often combining different therapies because they work in different ways to improve the health and appearance of your skin.
SkinPen Microneedling
SkinPen microneedling is one of our most effective treatments for improving skin quality from within. By creating thousands of microscopic channels in the skin, it stimulates the body’s natural healing response, encouraging the production of fresh collagen and elastin. Over time, this improves skin texture, firmness and overall resilience.
For clients with dehydrated, crepey skin or established static lines, a course of SkinPen treatments helps rebuild the collagen that has naturally declined with age. As collagen levels increase, the skin becomes firmer, smoother and healthier, allowing fine lines to soften naturally.
Microneedling is particularly beneficial for dehydration-related fine lines because it addresses the underlying problem. Rather than simply relaxing the muscles beneath the skin, it improves the skin’s own ability to retain moisture and repair itself, resulting in healthier skin that continues to improve long after treatment.
Skin Boosters and Polynucleotides
When dehydration is the main cause of fine lines and dull-looking skin, we often recommend skin boosters as part of the treatment plan. These injectable treatments deliver deep hydration directly into the skin, improving its quality from within rather than simply treating the surface.
At RiKo Aesthetics, we offer treatments such as Polynucleotides, Profhilo and Seventy Hyal to restore hydration, improve skin quality and support long-term skin health. Unlike topical moisturisers, which work primarily on the outer layer of the skin, these treatments deliver hydration where it is needed most, creating meaningful improvements in the skin’s structure, texture and overall appearance.
Polynucleotides offer benefits beyond hydration alone. They stimulate fibroblasts, the cells responsible for producing collagen and elastin, helping the skin repair itself naturally. This means the skin not only looks healthier and more hydrated after treatment, but continues to improve over the following weeks and months. For clients whose fine lines are largely caused by dehydration or declining skin quality, this is often one of the most effective treatments we can recommend.
A Combined Approach Often Delivers the Best Results
For many clients, the best outcome comes from combining treatments rather than relying on a single procedure.
A personalised treatment plan might include SkinPen microneedling to stimulate collagen production, skin boosters or polynucleotides to restore hydration, and anti-wrinkle injections to soften dynamic lines caused by facial movement.
This is not about recommending more treatments than you need. It is about recognising that different treatments address different aspects of the ageing process. Skin quality, hydration, collagen loss and muscle activity all contribute to how the face ages. Treating each concern appropriately allows us to achieve results that look more natural, last longer and better reflect your individual goals.
At RiKo Aesthetics, every treatment plan is tailored to the individual sitting in front of us. We will never recommend treatments simply for the sake of it. Equally, if we believe anti-wrinkle injections alone are unlikely to deliver the result you are hoping for, we will always explain why and discuss the alternatives. Our priority is helping you achieve the best possible outcome, not simply providing the treatment you initially requested.
How We Have This Conversation in Clinic
When a client books an appointment for anti-wrinkle injections but our assessment suggests that skin support should come first, or be carried out alongside treatment, we take the time to explain exactly what we have found.
We show you what we are seeing in your skin and explain the difference between dynamic and static lines. We discuss what anti-wrinkle injections can realistically achieve, where they have limitations, and how treatments that improve hydration and collagen can complement the final result.
Many clients arrive expecting a straightforward Botox appointment and leave with a much clearer understanding of why their skin looks the way it does. These conversations are never about selling additional treatments. They are about helping you invest in the treatments that are most likely to achieve the results you want.
One of the most rewarding moments in clinic is seeing a client understand that the concern they have been trying to treat for years has a different underlying cause. Once skin quality begins to improve, anti-wrinkle treatments often perform better, results appear more natural and clients feel far happier with their overall appearance.
Signs Your Skin May Benefit from Support Before Botox
If any of the following sounds familiar, it may be worth discussing skin support before, or alongside, anti-wrinkle treatment.
- Your Botox results do not seem to last as long as they used to.
- You have had anti-wrinkle injections, but you still feel that the lines are there.
- Your skin looks dull, tired or has lost its natural glow.
- You have fine, crepey lines that remain visible even when your face is relaxed.
- Your skin feels dry or dehydrated, despite using moisturisers regularly.
- You have lines that have become a permanent feature of your face, rather than only appearing when you make facial expressions.
None of these signs mean that Botox is not the right treatment for you. In many cases, anti-wrinkle injections remain an important part of the treatment plan. However, they often work best when the skin beneath them is healthy, hydrated and supported. Creating that foundation is always our first priority at RiKo Aesthetics because it allows every treatment that follows to achieve its full potential.
The Series in Full: What You Think You Need vs What You Actually Need
This article completes our four-part series exploring some of the most common treatment misconceptions we see in clinic. Each article looks at a situation where a client arrives expecting one treatment but, following a thorough consultation, discovers there may be a more appropriate approach.
Our aim has never been to persuade clients to have more treatment. Instead, we want to help people understand why a personalised assessment is so important. Every face is different, every concern has an underlying cause, and the best outcomes come from treating the cause rather than simply the symptom.
The series includes:
- Part 1: You Think You Need Lip Filler. You Might Actually Need Chin Filler.
- Part 2: You Think You Need Under-Eye Filler. You Might Actually Need Cheek Filler.
- Part 3: You Think You Need Nose Filler. You Might Actually Need Lip Filler.
- Part 4: You Think You Need Botox. You Might Actually Need Skin Support First.
The message throughout every article is the same. A thorough consultation with an experienced clinician who assesses the whole face, listens to your concerns and develops an individual treatment plan will almost always produce better, safer and more natural-looking results than choosing a treatment in isolation. That is what doctor-led aesthetics means in practice, and it is the standard of care we are committed to providing at RiKo Aesthetics.
Book Your Consultation
If you have been considering anti-wrinkle injections, or feel that your current treatments are no longer giving you the results you hoped for, we would love to help.
