Anti-Wrinkle Injections in Hemel Hempstead: The Complete Guide
By Dr Rianna, Co-founder of RiKo Aesthetics | Doctor-Led Aesthetics Clinic, Hemel Hempstead
Anti-wrinkle injections are the most requested treatment we offer at RiKo Aesthetics and the one that, when done well, makes the most consistently transformative difference to how a client feels about their face.
But “done well” is doing a lot of work in that sentence. Because anti-wrinkle treatment is not simply a case of injecting a muscle and waiting two weeks. The difference between a result that looks natural, refreshed, and entirely like you and one that looks overdone, flat, or frozen comes down entirely to the quality of the assessment, the precision of the placement, and the clinical judgement of the person holding the needle.
At RiKo Aesthetics, every anti-wrinkle appointment begins with a thorough consultation. We assess your facial anatomy, your muscle strength and movement, your skin quality, and your goals. We discuss what you’d like to achieve and give you our honest clinical opinion on what we recommend. And we treat every client as an individual because no two faces are the same, and a cookie-cutter approach to anti-wrinkle injections produces exactly the kind of cookie-cutter results that give the treatment a bad reputation.
This is the complete guide to anti-wrinkle injections at RiKo Aesthetics. What we treat, how we approach it, what to expect, and why doctor-led care makes all the difference.
How Anti-Wrinkle Injections Work
Anti-wrinkle injections use botulinum toxin (a purified protein) to temporarily relax specific muscles in the face and body. By reducing the movement of the muscle responsible for a particular expression line, the overlying skin is allowed to smooth, and the line becomes significantly less visible or disappears entirely.
The results are not immediate, they develop gradually over the first seven to fourteen days as the product takes full effect. This is why we offer every client a complimentary two-week review appointment, where we assess the result and, if needed, make any small adjustments to ensure the outcome is exactly right.
It is important to understand that anti-wrinkle injections address lines caused by muscle movement; what we call dynamic lines. For lines that are present even at rest (static lines) additional treatments such as SkinPen microneedling, skin boosters, or polynucleotides may be needed alongside anti-wrinkle injections to achieve the best possible result. We discuss this in detail in our blog post on what you think you need versus what you actually need: Botox vs skin support.

What We Treat: A Full Face and Neck Approach
One of the things that consistently surprises clients at RiKo Aesthetics is the breadth of what anti-wrinkle injections can address. We offer a comprehensive full face and neck anti-wrinkle service and we always assess every area of the face at consultation, not just the ones a client mentions.
The reason for this is simple: anti-wrinkle treatment works best when the whole face is considered. Treating one area in isolation without understanding how it relates to the areas around it can produce results that look unbalanced and can even create new concerns where none existed before.
Here is a guide to everything we treat and what each area achieves.

The horizontal lines that develop across the forehead from years of raising the eyebrows are among the most commonly treated and one of the areas where anti-wrinkle injections are most effective.
Careful placement and precise dosing are essential here. Too much product in the forehead can cause the brow to drop, creating a heavy, hooded appearance that clients find worse than the original lines. At RiKo Aesthetics, we always assess the relationship between the forehead and the brow before treating, and we dose conservatively to preserve natural lift and expression while smoothing the lines.

The vertical lines that form between the eyebrows (often called the “11 lines”) develop from years of frowning, concentrating, and squinting. They can give the face an unintentionally angry or tired appearance even when the face is at rest, and anti-wrinkle injections in this area are consistently one of the most impactful treatments we offer.
Softening the glabella creates a more open, approachable, and rested appearance and for many clients, it is the single change that makes the most difference to how others perceive them.

The fine lines that radiate from the outer corners of the eyes when smiling are an area where precision matters enormously. Treat too aggressively and the eye loses its natural warmth and expressiveness. Treat with the right amount in the right placement and the area softens beautifully; the eye still smiles, the lines simply become less pronounced.
At RiKo Aesthetics, our approach to crow’s feet always preserves natural eye movement and expression. We never aim for a result that looks flat or expressionless, we aim for one that looks like you on a very good day.

This is perhaps the most overlooked and underutilised area in anti-wrinkle treatment and one that we assess as standard for every client at RiKo Aesthetics.
We hear regularly from clients who have been having anti-wrinkle injections for years elsewhere and have never been offered a brow lift. When we explain what it is and what it can achieve, the reaction is almost always the same: why has nobody told me about this before?
The brow lift works by placing a small amount of anti-wrinkle product beneath the eyebrow to relax the orbicularis oculi (the circular muscle that surrounds the eye) and, when overactive, pulls the lateral brow downward. By gently relaxing this muscle, we allow the brow to lift naturally, giving the eye a more open, awake appearance and creating a subtle but beautiful elevation of the outer brow.
In plain terms: a couple of small injections beneath the eyebrow stop the muscle that drags the brow down from contracting so strongly. The brow lifts. The eye opens. The whole upper face looks fresher, more rested, and more youthful without anything looking done.
Most clients love this result. It is subtle, completely natural-looking, and the kind of difference that makes people say you look well without being able to identify why.

In my opinion, lower face anti-wrinkle treatment is the single most overlooked area in aesthetics and one of the most impactful things we can do for a face as it ages.
Most clients, when they think about anti-wrinkle injections, think about the upper face: forehead, frown, crow’s feet. And while these are important areas, it is the lower face that often holds the key to the most significant long-term results. Specifically, it is the muscles that pull the corners of the mouth downward and create the conditions for jowling that deserve far more attention than they typically receive.
The treatments we offer in the lower face include:
The DAO (Depressor Anguli Oris). The DAO is the muscle responsible for pulling the corners of the mouth downward creating the slightly downturned mouth that can make a face look sad, stern, or older than it is. By relaxing the DAO with a small amount of anti-wrinkle product, we allow the corners of the mouth to sit in a more neutral or slightly lifted position. The result is a face that looks more relaxed, more approachable, and simply happier at rest. Over time, treating this muscle also helps to slow the development of jowling because it is the repeated downward pull of the DAO, over years and decades, that contributes to the descent of the lower face.
The Mentalis. The mentalis is the muscle of the chin, and when overactive it creates the dimpled, pebbly chin texture (sometimes described as an orange peel effect), that many clients find distressing. Relaxing the mentalis smooths the chin surface and creates a cleaner, more refined lower face.
The Platysma Bands. The platysma is the broad, flat muscle of the neck, and as it ages it can develop visible vertical bands that run down the front of the neck (sometimes called “turkey neck”), creating an aged, stringy appearance. Anti-wrinkle injections into the platysma bands (known as a Nefertiti Neck Lift) relax this muscle, softening the bands and creating a smoother, more youthful neck contour.
A combination of DAO, mentalis, and platysma treatment works beautifully together — and in my clinical opinion, it represents the most cost-effective addition to any anti-wrinkle appointment in terms of the difference it makes to the overall result. Some clients choose to treat all three areas; others opt for the DAO alone, which even in isolation can make a remarkable difference to the lower face.
If you have been having anti-wrinkle injections for some time and feel that your results are good but not quite complete, that something is still slightly off about the overall balance of your face, there is a strong chance that the lower face is the missing piece.

Anti-wrinkle injections are not only a cosmetic treatment. At RiKo Aesthetics, we also offer botulinum toxin treatment for hyperhidrosis (the medical term for excessive sweating) in the underarms, hands, and feet.
Hyperhidrosis occurs when the nervous system triggers sweat glands to produce far more sweat than the body actually needs for temperature regulation. It can be profoundly confidence-affecting; impacting everything from clothing choices to professional situations to everyday social interactions. For many people who live with it, finding an effective treatment is genuinely life-changing.
Anti-wrinkle injections work for hyperhidrosis by temporarily blocking the nerve signals that activate the sweat glands in the treated area. The result is a dramatic reduction in sweating (clinical evidence consistently shows a reduction of 82 to 87% in treated areas), with results that last significantly longer than facial anti-wrinkle treatment.
For underarm hyperhidrosis (our most requested area), results typically last 6 to 9 months, with some clients experiencing relief for up to a year before a maintenance appointment is needed. For the hands and feet, results typically last 4 to 6 months. Results begin to appear within a few days of treatment, with the full effect visible at two weeks.
If excessive sweating is something you have struggled with, perhaps for years, perhaps having tried every clinical antiperspirant available, this treatment is worth a conversation. The difference it makes to quality of life is, for many clients, extraordinary.
How Long Do Anti-Wrinkle Results Last?
This is one of the questions we are asked most often, and the honest answer is that it varies, because results depend on a range of individual factors that differ from client to client.
Typically, facial anti-wrinkle results last 3 to 4 months before a maintenance appointment is needed. However, a number of factors influence this:
Metabolism. Clients with a faster metabolism tend to process the product more quickly, which can mean results wear off sooner. This is also why clients who exercise frequently, particularly those who do a significant amount of cardiovascular or high-intensity training, may find their results don’t last quite as long as those who are less active.
Muscle strength. Clients with particularly strong facial muscles, often younger or male clients, or those who are very expressive, may require slightly more product or more frequent appointments initially.
Skin quality and hydration. As we discuss in our blog post on Botox versus skin support, dehydrated skin can affect how anti-wrinkle results appear and how long they seem to last. Ensuring the skin is well supported with adequate hydration and collagen is always part of our approach.
Treatment history. One of the most reassuring things we tell clients who are new to anti-wrinkle injections is this: the more consistently you have the treatment over time, the less you tend to need. As muscles are repeatedly relaxed over months and years, they gradually weaken and require less product to achieve the same result. This means that long-term anti-wrinkle clients often find their appointments become less frequent and require lower doses, which is both more cost-effective and more natural-looking over time.
The Two-Week Review: Included for Every Client
At RiKo Aesthetics, every client who has anti-wrinkle injections with us receives a complimentary two-week review appointment as standard.
At this appointment, we assess how the product has settled, check that the result is even and balanced, and make any small adjustments needed to ensure the outcome is exactly right. This is not an afterthought — it is a fundamental part of how we deliver results we are proud of.
Anti-wrinkle injections are a precise treatment, but individual variation in how the product takes effect means that occasionally a small top-up in a specific area makes all the difference between a good result and a great one. The two-week review is where we ensure every client leaves with the latter.
Why Doctor-Led Anti-Wrinkle Treatment Matters
Anti-wrinkle injections are one of the most commonly performed aesthetic treatments in the UK – but they are not without risk when administered by someone without the right training, anatomical knowledge, and clinical judgement.
All botulinum toxin products are prescription-only and at RiKo Aesthetics, all prescription-only treatments are delivered within a doctor-led clinical model. Every patient is personally assessed by me, a GMC-registered doctor and I determine whether treatment is appropriate, discusses the benefits and risks, and prescribes the medication where clinically indicated.
This approach provides patients with the reassurance that the clinician responsible for prescribing their treatment is directly involved in their care from the outset and remains available throughout their treatment journey. Unlike some clinic models where prescribing is outsourced to an external clinician who may have no ongoing involvement after the prescription is issued. This means your assessment, treatment planning, aftercare, and management of any concerns or complications are delivered within the same trusted clinic, ensuring continuity of care, clear clinical accountability, and a consistently high standard of patient care.
We are proud to be one of Hertfordshire’s trusted doctor-led aesthetics clinics, treating clients from Hemel Hempstead, St Albans, Watford, Berkhamsted, and across the surrounding area.
