You Might Actually Need Cheek Filler.

Dark circles. Hollowness. That persistent look of tiredness that no amount of sleep or concealer seems to fix. If this sounds familiar, there’s a good chance you’ve looked into tear trough filler, the treatment specifically designed for the under-eye area, and wondered if it might be the answer.

It might be. But it also might not be.

At RiKo Aesthetics, one of the most important things we do in every consultation is assess not just the area a client is concerned about, but why that concern exists in the first place. And when it comes to the under-eye area, the answer is often found somewhere else entirely – in the mid-face, specifically the cheeks.

This is the second post in our series on what you think you need versus what you actually need. If you haven’t read the first on why you might think you need lip filler but actually need chin filler it’s worth a read alongside this one, because the principle is the same: the face works as a system, and treating one feature in isolation without understanding the whole picture rarely delivers the result a client is truly looking for.


Why the Under-Eye and the Cheek Are So Closely Connected

To understand why cheek filler can resolve an under-eye concern, you need to understand the architecture of the face, specifically, how the mid-face supports everything above and below it.

The cheeks sit in the centre of the face and act, in many ways, as the structural foundation of the lower eyelid and tear trough area. When the cheeks are full and well-supported, the transition between the lower eyelid and the cheek is smooth and gentle. There is no harsh shadow, no hollow, no pronounced groove running from the inner corner of the eye toward the cheek.

As we age, or as we lose volume in the face through weight loss, the cheeks descend and deflate. Fat pads that once sat high and full begin to drop. And as they do, they pull the skin of the lower eyelid downward with them, creating the hollowing, shadowing, and tired appearance that so many clients attribute to the under-eye area itself.

The tear trough (the groove that runs from the inner corner of the eye diagonally across the upper cheek) becomes more pronounced not necessarily because the tear trough itself has changed, but because the cheek beneath it has lost the volume that once supported it.

In this situation, placing filler directly into the tear trough without addressing the cheek is like papering over a crack without fixing the wall behind it. It may improve things temporarily, but without the underlying support, the result is unlikely to be as good as it could be and in some cases, adding volume to an area that is already under structural pressure can make the appearance worse.


How We Decide: Tear Trough Filler vs Cheek Filler

At RiKo Aesthetics, every client who comes to us with under-eye concerns goes through a thorough assessment before we recommend any treatment. We look at several things:

Skin quality in the under-eye area. The skin beneath the eye is the thinnest on the face, and its quality matters enormously when it comes to filler placement. Thin, crepey, or very fine skin in this area can mean that filler placed directly into the tear trough becomes visible, creating a bluish discolouration known as the Tyndall effect. In cases where skin quality is the primary concern, we may recommend polynucleotides or skin booster treatments to improve the quality of the skin itself before considering filler.

Where the volume loss is occurring. We carefully assess whether the hollowness and shadowing in the under-eye area is genuinely localised to the tear trough, or whether it is being caused or significantly worsened by volume loss in the mid-face. If the cheeks have descended or deflated, we will always discuss cheek filler as part of the solution, because addressing only the tear trough without the cheek will give a limited result.

Whether both are needed. In many cases, the answer is not either/or, but instead, both, in the right order. Restoring volume to the cheeks first provides the structural foundation, and if tear trough filler is still needed after that, it can then be placed with greater precision and a better outcome. We always assess and plan the full picture at consultation.


The Weight Loss Factor: When a Changing Face Needs a New Approach

One of the most significant and increasingly common reasons we see clients presenting with mid-face volume loss and pronounced under-eye hollowing is weight loss.

When we lose weight, we lose it everywhere, including the face. The fat pads of the mid-face, which provide the structural volume that supports the under-eye area and creates the smooth, rounded contour of a youthful cheek, can diminish significantly with weight loss. The result is a face that can look gaunt, hollowed, or older than it did before even as the body may look and feel healthier.

This is something we are seeing with increasing frequency at RiKo Aesthetics, particularly as GLP-1 medications, such as Ozempic and Mounjaro, have become more widely used for weight management across Hertfordshire and beyond. What is sometimes referred to as “Ozempic face” is precisely this phenomenon: rapid or significant weight loss leading to facial volume loss, mid-face descent, and a hollowed, tired appearance that clients are often distressed by.

If you have lost weight recently; whether through medication, lifestyle changes, or any other means, and have noticed that your face looks more tired, more hollow, or older than it did before, mid-face volume loss may well be a significant part of what you are experiencing. And in this case, cheek filler to restore the volume that weight loss has taken is often the most impactful treatment we can offer.

This is a conversation worth having at consultation, and one we are well-equipped to have. What Cheek Filler Can Achieve

When placed correctly by a doctor with a thorough understanding of facial anatomy, cheek filler can achieve results that go far beyond simply making the cheeks look fuller. In the context of under-eye concerns, it can:

  • Lift and support the tear trough area, reducing the appearance of hollowness and dark shadows without any filler placed directly into the under-eye
  • Restore the smooth, gradual transition between the lower eyelid and the cheek that gives a face a rested, refreshed appearance
  • Lift the mid-face gently, which can have a subtle but meaningful effect on the lower face and jawline as well
  • Restore the facial proportions that weight loss or ageing has altered, bringing the face back into balance

The results, when done well, are natural. Nobody should be able to identify what has been treated. They should simply notice that you look well, rested, and like yourself again.


And When Is Tear Trough Filler the Right Answer?

Tear trough filler absolutely has its place and at RiKo Aesthetics, we offer it as part of our treatment menu for clients for whom it is genuinely the right choice.

If the tear trough hollowing is localised and not significantly driven by mid-face volume loss, if the skin quality in the under-eye area is good enough to carry filler well, and if the overall facial structure is sound, tear trough filler can be a beautifully effective treatment that makes a meaningful difference to a persistently tired appearance.

The key is knowing when it is and isn’t appropriate and that is a clinical judgement that requires a proper assessment, not a decision made on the basis of what a client has read online or seen on social media.
